=== Action on the cell === After insulin enters the bloodstream, it binds to a membrane-spanning receptor tyrosine kinase (RTK). == Hereditary copper metabolic diseases == Several rare genetic diseases (Wilson disease, Menkes disease, idiopathic copper toxicosis, Indian childhood cirrhosis) are associated with the improper use of copper in the body. All of these diseases involve mutations of genes containing the genetic codes for the production of specific proteins involved in the absorption and distribution of copper. When these proteins are dysfunctional, copper either builds up in the liver or the body fails to absorb copper. These diseases are inherited and cannot be acquired. Adjusting copper levels in the diet or drinking water will not cure these conditions (although therapies are available to manage symptoms of genetic copper excess disease). The study of genetic copper metabolism diseases and their associated proteins are enabling scientists to understand how human bodies use copper and why it is important as an essential micronutrient. The diseases arise from defects in two similar copper pumps, the Menkes and the Wilson Cu-ATPases. === The discovery of copper-histidine in human blood and invention of Menkes disease treatment === Sarkar discovered copper-histidine in human blood in 1966 and recognized it as a biological form by which copper, an essential element to sustain life, is transported in blood. In 1976, Sarkar proposed that a baby with Menkes disease receive copper-histidine via subcutaneous injection. This was the world's first Menkes patient to receive copper-histidine therapy. Children with this disease are now living longer and reaching adulthood with copper-histidine treatment. Sarkar did not patent copper-histidine; he intended that it be readily available to Menkes patients at a reasonable cost. The formulation and detailed compounding procedure for the preparation of copper-histidine is freely available by SickKids Pharmacy to physicians and hospital pharmacies around the world upon request. == Treatment == Avoid cold weather. Treat the underlying lymphoma. No cold drinks; all drinks should be at room temperature (or above). Requires heater to maintain temperature in cold places. Treatment with rituximab has been described. The treatment of cold agglutinin disease depends on many factors including the severity of the condition, the signs and symptoms present in each person, and the underlying cause. For example, in those affected by secondary cold agglutinin disease, it is important to diagnose and treat the underlying condition which may include certain types of cancer; bacterial, viral, or parasitic infections; and/or other autoimmune disease. People with few symptoms and/or mild anemia may not require any specific treatment. These cases are often managed by simply avoiding exposure to the cold. In severe cases of hemolysis, medical interventions may be necessary. Rituximab (an antibody that selectively reduces specific types of immune cells) is effective in about 60% of cases of severe cold agglutinin disease. Medical researchers have found the response to rituximab is seen on average within 1 to 2 months of treatment and the effect of the treatment lasts for about 1 to 2 years. Rituximab may be used after a second and even a third relapse, however the success rate is less. Combined treatment with rituximab and fludarabine has resulted in higher response rates (76% of cases) and longer periods of remissions (on average, 6.5 years).
