== Causes == A combination of poorly-maintained or non-functional facilities, a disregard of safety standards, and an under-trained workforce made it possible for water to leak into the MIC tanks in the absence of properly working safety systems. Union Carbide held the view that concerted human effort is required for water to enter the tank, and argued that such action is conducted by a rogue employee, who hooked a water hose directly to an empty valve on the side of the tank. Union Carbide alleges that the Indian government took extensive actions to hide this possibility in order to attach blame to UCC. Theories differ as to how the water entered the tank. At the time, workers were cleaning out a clogged pipe with water about 120 metres (400 ft) from the tank. They claimed that they were not told to isolate the tank with a pipe slip-blind plate. The operators assumed that owing to bad maintenance and leaking valves, it was possible for the water to leak into the tank. This water entry route could not be reproduced despite strenuous efforts by motivated parties. UCC claims that a "disgruntled worker" deliberately connecting a hose to a pressure gauge connection was the real cause. Early the next morning, a UCIL manager asked the instrument engineer to replace the gauge. UCIL's investigation team found no evidence of the necessary connection; the investigation was totally controlled by the government, denying UCC investigators access to the tank or interviews with the operators.
==== Cervical cancer ==== Oncogenic types of the human papillomavirus (HPV) are known to hijack cellular ubiquitin-proteasome pathway for viral infection and replication. The E6 proteins of HPV will bind to the N-terminus of the cellular E6-AP E3 ubiquitin ligase, redirecting the complex to bind p53, a well-known tumor suppressor gene whose inactivation is found in many types of cancer. Thus, p53 undergoes ubiquitylation and proteasome-mediated degradation. Meanwhile, E7, another one of the early-expressed HPV genes, will bind to Rb, also a tumor suppressor gene, mediating its degradation. The loss of p53 and Rb in cells allows limitless cell proliferation to occur.
== Pulsatile insulin and the liver == Normally, insulin is secreted from the pancreas in pulses into the portal vein which brings blood into the liver in variable amounts, closely related to ingestion of meals. For induction and maintenance of insulin-dependent enzymes essential for glucose metabolism in the liver (e.g. hepatic glucokinase, phosphofructokinase, and pyruvate kinase), the hepatocytes require a defined insulin level (200-500 μU/ml in the portal vein) concomitant with high glucose levels (which acts as a bimolecular signal). In non-diabetic subjects, portal insulin concentrations are twofold to threefold greater than those in the peripheral circulation. During the first pass through the liver, 50% of the insulin is removed, strongly insinuating that the liver is the principal metabolic target organ of the gastrointestinal tract and the pancreas. The insulin retained by the hepatocytes may itself be essential for the long-term effects of insulin on hepatic glucose metabolism as well as growth and de novo enzyme synthesis. Following oral glucose intake, the liver accounts for an equal or greater portion of total net glucose uptake compared to the periphery. Insulin exerts pivotal control of glucose levels through its ability to regulate hepatic glucose production directly or indirectly. The traditional subcutaneous (S.C.) insulin administration regimens used by diabetic patients fails to capture the pulsatile nature of natural insulin secretion and does not reach high enough insulin concentrations at the hepatocyte level (e.g., 10 U regular insulin injected S.C.
== Administration == Piperacillin is not absorbed orally, and must therefore be given by intravenous or intramuscular injection. It has been shown that the bactericidal actions of the drug do not increase with concentrations of piperacillin higher than 4-6× MIC, which means that the drug is concentration-independent in terms of its actions. Piperacillin has instead shown to offer higher bactericidal activity when its concentration remains above the MIC for longer periods of time (50% time above MIC showing the highest activity). This higher activity present in continuous dosing has not been directly linked to clinical outcomes, but however does show promise of lowering possibility of resistance and decreasing mortality. Extending the time of piperacillin-tazobactam infusion allows the drugs to maintain the necessary concentrations needed within the body to prevent bacterial growth, enhancing bactericidal activity. The studies supporting this theory generally administered ~3.375 g of piperacillin-tazobactam every 8 hours during a 4-hour infusion, while for organisms with higher minimum inhibitory concentrations, ~4.5 g of piperacillin-tazobactam was administered every 6 hours during a 3-hour infusion. The recommended doses provided by the BNFC for infants with hospital-acquired infections are 90 mg/kg every 8 hours for infants, a maximum of 4.5 g every 6 hours for children, and 4.5 g every 8 hours for children aged 12 and above. A dosage of 90 mg/kg every 6 hours is suggested for infants and children diagnosed with neutropenia.

After the start of the Arab Spring, in 2011, Gaddafi spoke out in favour of Tunisian President Zine El Abidine Ben Ali, then threatened by the Tunisian Revolution. Unlike Tunisia or Egypt, Libya was largely religiously homogeneous and had no strong Islamist movement, but there was widespread dissatisfaction with the corruption and entrenched systems of patronage, while unemployment had reached around 30 percent. According to Thomas Molloy, writing in the Irish Independent, "It had no qualms about selling drink to Catholics but it did everything it could to avoid employing them until the 1960s." Guinness thought they brewed their last porter in 1973. In the 1970s, following declining sales, the decision was taken to make Guinness Extra Stout more "drinkable". The gravity was subsequently reduced, and the brand was relaunched in 1981. Pale malt was used for the first time, and isomerised hop extract began to be used. In 2014, two new porters were introduced: West Indies Porter and Dublin Porter. Guinness acquired The Distillers Company in 1986. This led to a scandal and criminal trial concerning the artificial inflation of the Guinness share price during the takeover bid engineered by the chairman, Ernest Saunders. A subsequent £5.2 million success fee paid to an American lawyer and Guinness director, Tom Ward, was the subject of the case Guinness plc v Saunders, in which the House of Lords declared that the payment had been invalid. In the 1980s, as the IRA's bombing campaign spread to London and the rest of Britain, Guinness considered scrapping the harp as its logo. In 1997 the company merged with Grand Metropolitan to form Diageo.
